2012 GPCI comparison

Quick summary: big movement in the GPCI values this year. The
southern/mid-west gets killed (WV, OK, MO, etc.).

Background: The GPCI is a small piece of the RVU calculation that many people
overlook each year. It's a geographical adjustment can change by more than
10% each year above and beyond any conversion factor or code changes.
