"Meet the doctor" Appointments

How many of you offer "Meet the Doctor" appointments for families new  
to the office?

Right now we are not billing for this appointment.  This impacts  
available sick appointments for the day and the bottom line.

At the least, CAN we bill for these appointments?
Can we bill if the baby is not born yet and the parents are just  
interviewing?  CPT talks about consultation with the family and/or  
patient.  Is a family a family if the patient is not born yet, or  
does family start with the patient's birth?

What is the CPT and ICD9 for a "meet the doctor"? (baby born or unborn).

What I do is this:
Prenatal visits are free. i figure you are going to
committ to several days of inpatient care (longer if you
wind up with a NICU stay) from me so you had better like
me.
Post natal visits are charged based upon time spent
consulting. Most of these visits tend to be free consults
otherwise. I say schedule a well visit with me, if you
don't like me, don't come back. either way you are paying
for my time.

These come up so rarely it's hard to remember if we've charged but I
think not. For families with children who want to look around the
office manger gives a tour and answers questions and maybe a doctor
says hi for a minute or two. If it is a "how would you deal with
this compared to our old doctor"... that's a visit.
Prenatals are free and in the evening. Almost always it is a first
time mom at the prenatal. Once in a while someone who lives locally
but has been taking their older kids to their previous pediatrician,
but now is having another baby and feels they've had it with
traveling. A visit to see us is still a prenatal and if someone who
just has older children want's to talk for a while, we'll do that in
the same time frame.

Most of our "meet the doctor" appointments are expecting parents. We
heard you can bill for this visit. What is the ICD9 and CPT to use
when you are not YET the PCP because the infant is not born yet
(managed care)?
