Patient Information/Face Sheets

We are revising our face sheet (patient demographic/information sheet) I'm
just wondering if anyone would be interested in sharing your office's patient
demographic sheet with me? My fax # is 231-935-0562.

I am also interested in knowing how often your front desk staff update patient
information in the computer and on paper? Once per year? Every 6 months ? Do
you hold the patient at the window to do this or just have them fill out the
paper forms ?

We have problems with patients signing in on our log and then sitting down
right away. We have one person at check in and am trying to determine if our
process could be improved vs. hiring an additional staff member.
